Clinical Pharmacist (CP)

Role Description

Clinical pharmacists work in a patient-facing role as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks.

Clinical Pharmacists should be part of a professional clinical network.

What can a Clinical Pharmacists offer to patients in a Primary Care setting?

Primary Care Clinical Pharmacists provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long-term conditions, while addressing both the public health and social care needs of patients in the GP practices that make up the PCN.

Qualifications

Master of Pharmacy (MPharm) degree (or equivalent) approved by General Pharmaceutical Council.

Additional Qualifications/Training Required for Primary Care

To have completed or be enrolled on the Centre for Pharmacy Postgraduate Education (CPPE) Primary Care Pharmacy Education pathway (PCEP) 18-month pathway, unless they have already completed the Medicines Optimisation in Care Homes (MOCH) pathway or the Clinical Pharmacists in GP Practice Scheme
Independent Prescribing (IP) qualification (if not already an IP training to be made available to become a prescriber)

Regulatory Body

Pharmacist Registration with the General Pharmaceutical Council GPhC.

Supervision Requirements

For a clinical pharmacist, a minimum of one supervision session per month by a senior clinical pharmacist. The senior clinical pharmacist must receive a minimum of one clinical supervision session every three months by a GP clinical supervisor.
Each clinical pharmacist to have access to an assigned clinical supervisor who should be a GP for support and development.
