GP Mentee Application Form

Applications are invited from GPs working across Surrey who would like to have a  GP Mentor .

• Are you on the GP Fellowship scheme?
• Are you a mid-career GP and need some advice on career choices including deciding to take on a partnership role?
• Are you a new GP partner, including those on the New to Partnership Payment Scheme?
• Are you a GP on the National GP Retention Scheme who is transitioning to a regular GP role outside of the scheme?
• Are you a locum GP looking to take on a salaried role?
• Are you a GP thinking of returning to the profession?

If any of these scenarios apply and you would like mentoring support from an experienced Surrey GP Mentor then please complete this application form. 

Mentoring appointments may be conducted virtually or face to face and will typically be 6 appointments spread over a year. 

Each mentoring appointment will be between 1.5- 2 hours and arranged at a time that is mutually convenient to both the GP Mentor and Mentee.
